Squid Configuration

I am a complete squid noob having just downloaded it 45 minutes and 
stumbling through the install.  I think I got everything running, but I 
can't get proxied connections to run.  Here is my access.log output if 
anyone has an idea.

I am thinking that my outbound connections might be being denied, but I 
thought that my hosting service wasn't filtering outbound, just the 
inbound.  I am probably wrong, but I want to double check.
